MPHA’s assisted living and Housing with Services (HWS) programs increase housing choices for MPHA residents with special needs. Without these programs, frail, elderly and disabled residents and applicants would not have public housing as a housing option. These programs also reduce costs and provide greater cost effectiveness in the delivery of housing and related services. Assisted living/housing with services clients avoid nursing home placements, remain in units longer, receive supportive services and enjoy independent living. Assignment of assisted living by slot as opposed to hard unit assignment, allow residents to remain in their own apartment instead of having to move to receive needed services.

Cedar High Apartments (People Incorporated- Epilepsy and Mental Health Services); 630 Cedar Avenue South Minneapolis, MN 55454
This program serves adults who have a primary diagnosis of epilepsy and are capable of living independently with support services are provided 24 hour emergency assistance and monitoring services by People Inc. Clients are taught independent living skills and can receive individual counseling and support services necessary to attain their goals.
Heritage Commons at Ponds Edge (Assisted Living Program, Augustana); 350 Van White Memorial Boulevard, Minneapolis, MN 55405
Provides services to elderly adults requiring assistance with independent living. Services provided are assistance with personal care, weekly housekeeping, and case management, two nutritionally balanced congregate meals daily, emergency response-24 hours a day, assistance with medications, bathing assistance, planned recreational activities and on-call nursing. Maximum Client Capacity:40 (Forty)
Lyndale Manor (Assisted Living Program, Volunteers of America); 600 – 18th Avenue North, Minneapolis, MN 55411
This 3-5 story midrise with 239 units; provides programs for elderly adults & adults with disabilities who need assistance – personal care, case management, two nutritionally balanced congregate meals daily, weekly housekeeping assistance, emergency response 7:00 a.m. to 11:00 p.m., assistance with medications, weekly bathing assistance, planned recreational activities and on-call nursing. Maximum client capacity: 30 (Thirty)
Parker Skyview (Assisted Living Program, Volunteers of America); 1815 Central Avenue NE, Minneapolis, MN 55418
This 25 story, 333 unit building provides services for elderly adults, as well as adults with disabilities. Services provided are assistance with personal care, case management, two nutritionally balanced congregate meals daily, weekly housekeeping, emergency response 7:00 a.m. to 11:00 p.m.; assistance with medications, weekly bathing assistance, planned recreational activities and on-call nursing. Maximum client capacity: 40 (Forty)
This seven story, 42 unit building, provides services for elderly adults. Services include: onsite nursing , on call nursing after hours, assistance with personal care, case management, three nutritionally balanced congregate meals, weekly housekeeping, weekly laundry, 24 hour emergency response, assistance with medications and planned recreational activities. Maximum Client Capacity:42 (Forty-two)
Third Avenue Towers (Accessible Space Inc. – Housing with Services); 1707 – 3rd Avenue South, Minneapolis, MN 55404
This program provides housing and services to traumatically brain injured, mobility impaired, and physically disabled adults who require specialized support services which include: case management, encouragement in the development of social, employment related and/or independent living skills, personal care services, assistance with cooking and cleaning and 24 hour staff coverage. Each participant is given an in-depth assessment and individual care plans are developed to meet their specific needs.
Thomas T. Feeney Manor (Enhanced Assisted Living and Memory Care Program, Augustana Care); 901 4th Avenue North, Minneapolis, MN 55405
This 4 story 48 unit ‘smoke free’ development provides a more supportive environment with a culture of person-centered care, services tailored to residents’ unique needs and preferences with apartments that are designed specifically for individuals experiencing cognitive changes. Services include: Assessment by Registered Nurse, diabetic care options including sliding scale insulin, three meals per day, weekly light housekeeping, weekly laundry services, monthly vitals, emergency response system, social and life enrichment programs and home health services 24 hours a day 7 days a week. Memory Care includes all of the above as well as secured areas, cognitive screening performed by Occupational Therapist to determine the proper level of care and staff specifically trained in the care of dementia residents. This development is specifically designed for adults over 55 who need enhanced assisted living and memory care services.
